Initial Comments:
The facility advised of a legal name change via correspondence dated July 14, 2021. The facility will be entitled Harborcreek Youth Services (legal name/entity number 868275) and Harborcreek Youth Services - Conway House (working name with location indicator). This change was filed with The PA Department of State as received on March 3, 2021.
Harborcreek Youth Services is in compliance with the requirements of 42 CFR, Part 483, Subpart G Requirements for Psychiatric Residential Treatment Facilities.
